Patient Portal and Privacy Rights

Any minor thirteen years or older may request and receive outpatient treatment without the consent of the minor’s parent. Parental authorization, or authorization from a person who may consent on behalf of the minor pursuant to RCW 7.70.065, is required for outpatient treatment of a minor under the age of thirteen. This includes access to online medical records (patient portal).

If you would like to set up an account for our MyHealthRecord patient portal, please send us a message or call (360) 456-1600 and choose option 1.

We will send you an invitation to register. Please note that the portal account holder’s (parent/guardian’s) name and birthdate (not the patient’s) are required to log in.
